ENROLLMENT PROCEDURE
The following items must be turned in before your child attends preschool
The following items must be turned in before your child attends preschool
- An Enrollment Application
- Fully completed Emergency and Immunization Information Card (Blue
Card) - A copy of your child’s immunization record with doctor or clinic
stamp/signature - Signed Parent Handbook Receipt
- Completed Phone Authorization for Alternate Pick-Up Form
- $100 registration fee plus first month’s tuition.
